Summarise the arrangement under review, its purpose, the main findings, and the proposed outcome. State the extent of any changes and the principal reasons for them. For a new arrangement, summarise what is proposed and why.

Set out what the decision-maker is asked to note or approve. Use wording that matches the person’s or body’s authority. Identify the proposed arrangement or changes, any conditions, and when they should take effect.
